Take on a vital role delivering quality care as an experienced and compassionate Home Health Registered Nurse. In this contract position, you will provide skilled nursing services to patients in their homes, ensuring continuity, safety, and a patient-centered approach throughout the care experience. This opportunity is ideal for a self-motivated RN who thrives working independently, is highly organized, and holds the highest clinical standards and dedication to patient communication.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
Role Highlights:
- Full-time, 13-week contract assignment
- Field-based with daily travel to patient homes (mileage reimbursement included)
- Caseload expectation: 2532 points per week (typically 35 patient visits per day)
- Utilizes Home Care Home Base (HCHB) electronic documentation and OASIS assessment tools
Key Responsibilities:
- Perform comprehensive OASIS assessments and implement customized care plans
- Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient condition, communicate changes promptly with the care team and physicians
- Educate patients and families on diagnoses, medications, and preventive care
- Ensure all documentation is accurate and timely using HCHB
- Collaborate closely with therapists, social workers, and home health aides
- Attend case conferences, staff meetings, and further education when needed
- Follow all regulatory and agency documentation guidelines
Qualifications:
- Active RN license in good standing
- At least 1 year of recent Home Health (field) experience
- Proficient in OASIS and Home Care Home Base (HCHB)
- Strong clinical, assessment, and critical-thinking skills
- Valid drivers license, auto insurance, and reliable transportation
- BSN and experience managing 2532 weekly points preferred
- Knowledge of Medicare and compliance standards a plus
