Bring your nursing expertise to the forefront as a Home Health Registered Nurse delivering high-quality, compassionate care in a field-based setting. This 13-week contract offers the opportunity to work closely with patients and their families in their homes, creating meaningful connections while ensuring the highest standards of care.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
In this role, youll manage a caseload averaging 2532 points per weektypically three to five patient visits per daywith a focus on comprehensive patient assessments and individualized care planning. Leveraging your proficiency in Home Care Home Base (HCHB) and OASIS, youll be responsible for documentation, coordination, and adherence to both regulatory requirements and best practice standards.
Key Responsibilities:
- Conduct thorough OASIS assessments and develop personalized care plans for each patient.
- Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient status, communicate changes to the care team, and adapt care as needed.
- Provide education to patients and families on treatment plans, medications, and health maintenance.
- Maintain timely and accurate electronic documentation using HCHB.
- Collaborate with an interdisciplinary team to ensure seamless, coordinated patient care.
- Participate in case conferences, team meetings, and continuing education.
Qualifications:
- Active RN license in good standing
- At least one year of recent home health nursing experience
- Proficient with HCHB and OASIS
- Strong assessment, communication, and organizational skills
- Valid drivers license, reliable transportation, and auto insurance
- BSN and Medicare home health knowledge preferred
