Bring your home health expertise to a rewarding nursing role supporting patients in their own homes. This position is ideal for a dedicated Registered Nurse (RN) seeking a contract opportunity with a focus on autonomy, patient-centered care, and professional growth.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
As a home health RN, youll deliver skilled care across Kansas City, KS, managing a caseload of 2532 points per week (typically 35 patient visits per day). Your day-to-day work will be field-based, assisting patients within their residences and ensuring the highest quality of clinical and educational support.
Responsibilities:
- Complete comprehensive OASIS assessments and develop tailored care plans
- Provide direct nursing interventions: wound care, medication administration, IV therapy, and chronic disease management
- Evaluate patient condition, monitor progress, and promptly update care teams and physicians
- Educate patients and families on treatment plans, medications, and preventive strategies
- Document care thoroughly using the Home Care Home Base (HCHB) electronic system
- Collaborate with physical therapists, occupational therapists, social workers, and aides for comprehensive care coordination
- Participate in case conferences, meetings, and ongoing education
- Maintain compliance with all regulatory standards for documentation and delivery of care
Qualifications:
- Active RN license (in good standing in Kansas)
- At least 1 year of home health experience; field experience strongly preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS documentation required
- Strong clinical assessment and problem-solving skills
- Reliable transportation, valid drivers license, and auto insurance
- BSN preferred
- Familiarity with Medicare and home health compliance is a plus
