Provide expert nursing care to patients in their homes as a Home Health Registered Nurse (RN) in Kansas City, KS. This full-time, 13-week contract position offers a rewarding opportunity to deliver patient-centered care while managing a caseload of 2532 points per week, typically involving 35 patient visits daily. Work independently within a supportive interdisciplinary team, focusing on quality outcomes for patients in a field-based setting.
Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress and communicate changes promptly with healthcare teams and physicians.
- Educate patients and families on disease management, treatment plans, and preventive care.
- Accurately document all care activities using the Home Care Home Base (HCHB) electronic system.
- Collaborate with therapists, social workers, and aides to ensure seamless coordinated care.
- Participate in case conferences, staff meetings, and continuing education sessions.
- Comply with all regulatory requirements and agency standards for documentation and care delivery.
Qualifications:
- Active and valid RN license in the practicing state (good standing).
- Minimum one year recent home health nursing experience, with field experience preferred.
- Proficiency in HCHB and OASIS required.
- Strong clinical assessment, critical thinking, and organizational skills.
- Reliable vehicle, valid drivers license, and auto insurance.
- Ability to work independently and manage a flexible schedule. Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience managing a caseload of 2532 points weekly.
- Knowledge of Medicare and home health compliance standards.
Schedule:
- Monday through Friday, 8:00 AM to 5:00 PM.
- Daily travel within assigned territory with mileage reimbursement provided.
