An experienced Home Health Registered Nurse (RN) is sought to provide skilled nursing care to patients within their homes. This role centers on delivering patient-focused, high-quality nursing services, performing thorough assessments, and managing individualized care plans. Job Type: Full-time, 13-week contract
Schedule:

• Monday Friday
• 8:00 AM 5:00 PM

Responsibilities:

• Perform comprehensive OASIS assessments and develop personalized care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize condition changes, and communicate promptly with care teams and physicians.
• Educate patients and families on disease processes, treatments, medications, and preventive care.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides to ensure coordinated care.
• Participate in case conferences, staff meetings, and continuing education.
• Adhere to all regulatory and agency standards for patient care and documentation.

Qualifications:

• Active RN license in the state of practice, in good standing
• Minimum of 1 year recent Home Health experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tool
• Strong clinical assessment and critical thinking skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to work independently and manage a flexible field-based schedule
• Excellent communication, time management, and organizational skills

Preferred:

• BSN degree
• Experience managing 2532 patient points per week
• Knowledge of Medicare and home health compliance standards
