An opportunity is available for a skilled and compassionate Registered Nurse specializing in Home Health to deliver patient-centered care within a supportive field-based setting. In this role, you will visit patients in their homes to provide direct nursing care, conduct clinical assessments, develop individualized plans, and work collaboratively with an interdisciplinary healthcare team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active RN license in good standing for the state of practice
- Minimum of 1 year of recent home health experience (field-based experience preferred)
- Proficiency with Home Care Home Base (HCHB)
- OASIS assessment experience required
- Reliable transportation, valid drivers license, and active auto insurance
- Strong communication, organizational, and time management skills
- Ability to work independently and demonstrate sound clinical judgment
Preferred:
- BSN degree
- Previous caseload management of 2532 points per week
- Familiarity with Medicare and home health compliance standards
Primary Responsibilities:
- Complete OASIS assessments and develop tailored patient care plans
- Deliver skilled nursing care including wound care, medication management, IV therapy, and chronic disease monitoring
- Evaluate and document patient progress, identifying and communicating changes in patient condition
- Educate patients and families on disease processes, treatment compliance, medications, and preventive healthcare measures
- Maintain thorough, timely documentation using Home Care Home Base (HCHB)
- Coordinate care with therapists, medical social workers, and home health aides
- Attend case conferences, staff meetings, and engage in ongoing professional development
- Adhere to all regulations, policies, and standards for quality home health care
