Job Description
Deliver high-quality, patient-centered care as a Home Health Registered Nurse, focusing on skilled nursing services in patients homes. In this field-based position, you will make a significant impact by supporting patients and families with clinical expertise and compassionate support while working autonomously within a flexible, MondayFriday schedule.
Schedule and Setting:
- Full-time, 13-week contract
- MondayFriday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (approximately 35 patient visits per day)
- Daily travel within assigned territory (mileage reimbursement provided)
- Field-based, visiting patient homes
Key Responsibilities:
- Complete comprehensive OASIS assessments and establish patient-specific care plans
- Deliver skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management
- Monitor and report changes in patient conditions, coordinating with medical providers and interdisciplinary teams
- Educate patients and families about disease management, medications, and preventive care
- Accurately document care using Home Care Home Base (HCHB)
- Collaborate with physical/occupational therapists, social workers, and aides for holistic care
- Participate in clinical meetings and professional development as appropriate
- Ensure all care and documentation complies with regulatory and agency standards
Qualifications:
- Active RN license (in good standing) in the state of practice
- At least 1 year of recent experience in Home Health (fieldwork preferred)
- Efficient in Home Care Home Base (HCHB) and OASIS assessments
- Excellent clinical assessment, critical thinking, and organizational skills
- Reliable transportation, current drivers license, and auto insurance
- Strong communication and ability to work independently
- BSN preferred
- Familiarity with Medicare and home health compliance is a plus
- Experience managing 2532 points per week strongly preferred
