Deliver skilled nursing care in patients homes as an experienced Home Health Registered Nurse (RN) focused on comprehensive, patient-centered care. This full-time contract offers a dynamic field-based role requiring clinical expertise, strong communication, and independent decision-making to optimize patient outcomes across a varied caseload.

Responsibilities:

• Conduct in-depth OASIS assessments and develop tailored care plans for patients in home settings.
• Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress and promptly report changes to interdisciplinary care teams and physicians.
• Educate patients and families on treatment regimens, medications, and preventive health strategies.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate care.
• Participate in case conferences and ongoing professional education, adhering to regulatory standards.

Qualifications:

• Active RN license in good standing for New Jersey.
• Minimum of 1 year recent home health nursing experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments required.
• Reliable transportation, valid drivers license, and auto insurance for daily patient visits (mileage reimbursed).
• Strong critical thinking, clinical assessment, communication, and organizational skills.
• BSN preferred.
• Familiarity with Medicare and home health compliance standards advantageous.

Schedule & Contract Details:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract with potential for extension
• Caseload expectation: 2532 points per week (~35 patient visits/day)
