Job Description
An experienced and compassionate registered nurse is needed to deliver skilled care to patients in their homes. This field-based role is vital for supporting patient recovery, education, and independence through clinical nursing expertise and patient-centered service.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract
- Caseload: 2532 points per week (typically 35 patient visits per day)
- Daily travel within the assigned territory with mileage reimbursement
Essential Duties:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Administer skilled nursing interventions (wound care, medication management, IV therapy, chronic disease support)
- Monitor patient status, promptly communicate changes to interdisciplinary teams, and adjust care plans as needed
- Provide patient and family education regarding disease processes, medications, and preventive healthcare
- Maintain timely, accurate documentation using Home Care Home Base (HCHB)
- Collaborate with therapists, social workers, and aides to ensure cohesive care
- Attend case conferences and participate in continuing education
- Ensure care and documentation comply with all regulatory standards
Qualifications:
- Active RN license (in good standing)
- At least one year of recent home health nursing experience (field experience preferred)
- Proficiency in Home Care Home Base (HCHB)
- Experience with OASIS assessment tool
- Reliable transportation, current drivers license, and auto insurance
- Strong clinical judgment, critical thinking, and communication skills
- Ability to work independently in the field, manage time, and organize a flexible caseload
- Preferred: BSN, prior experience managing 2532 points weekly, knowledge of Medicare/home health compliance
