Job Description
An experienced and compassionate Registered Nurse is needed to deliver exceptional care in a home health setting. This contract opportunity offers a rewarding environment where youll provide skilled nursing care to patients in their homes and play a vital role in supporting their recovery and ongoing health.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract
- Caseload of 2532 points per week (typically 35 patient visits per day)
- Daily travel is required within your territory; mileage reimbursement provided
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress, identify changes, and communicate promptly with healthcare team members
- Educate patients and families on disease processes, care plans, and preventive health strategies
- Accurately document all care activities in Home Care Home Base (HCHB)
- Collaborate with a multidisciplinary team to ensure high-quality, coordinated care
- Participate in case conferences, staff meetings, and professional development activities
- Adhere to all agency and regulatory documentation and care standards
Qualifications:
- Active RN license (in good standing) for the state of practice
- At least 1 year of recent home health experience (field experience preferred)
- Proficient with Home Care Home Base (HCHB) and OASIS assessments
- Reliable vehicle, valid drivers license, and current auto insurance
- Excellent clinical, communication, and time management skills
- BSN and experience managing a 2532 point weekly caseload are preferred
- Knowledge of Medicare and home health compliance standards is an advantage
