As a Home Health Registered Nurse (RN), youll work independently in a field-based capacity, delivering personalized care while ensuring continuity and clinical excellence. This contract position is ideal for self-motivated RNs dedicated to patient-centered care and quality outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Provide skilled nursing including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, quickly identify changes in condition, and communicate with the interdisciplinary team.
- Educate patients and families on disease processes, preventive care, medications, and treatment plans.
- Document all care accurately using Home Care Home Base (HCHB).
- Collaborate with therapy, social work, and aide teams to ensure coordinated and thorough care.
- Attend case conferences, participate in staff meetings, and engage in ongoing education.
- Maintain compliance with home health and regulatory standards.
Required Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum of 1 year recent, hands-on home health nursing experience.
- Strong proficiency in OASIS and Home Care Home Base (HCHB) systems.
- Reliable transportation, drivers license, and car insurance.
- Excellent clinical assessment, communication, and organizational skills.
- Ability to manage a flexible, independent home visit schedule.
Preferred Credentials:
- BSN degree.
- Experience carrying a 2532 point caseload weekly.
- Familiarity with Medicare and home health compliance regulations.
