Experienced and compassionate Registered Nurses are needed for a contract opportunity delivering skilled care in a home health setting. As a Home Health RN, you'll play a key role in supporting patient independence and recovery, providing direct nursing care in patient homes while collaborating with an interdisciplinary team to ensure optimal outcomes.

Hours & Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (35 patient visits per day)
• Field-based with daily travel within your assigned territory (mileage reimbursement provided)

Responsibilities:

• Conduct comprehensive OASIS assessments and create individualized care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress and promptly communicate any changes to care teams and physicians.
• Provide education to patients and their families on disease processes, care plans, medications, and preventive strategies.
• Accurately document all care and communication in Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for coordinated care.
• Attend case conferences, staff meetings, and pursue ongoing professional education when necessary.
• Maintain compliance with agency policies and regulatory guidelines.

Qualifications:

• Active RN License (must be in good standing)
• Minimum 1 year recent Home Health experience (field experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, valid drivers license, and auto insurance
• Strong clinical assessment, communication, and organizational skills
• BSN preferred
• Experience managing a caseload of 2532 points per week and knowledge of Medicare compliance is a plus
