Take your nursing expertise into the community by providing essential care as a Home Health Registered Nurse. This contract role offers the chance to build meaningful patient relationships while delivering skilled nursing in patients homes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Perform comprehensive OASIS assessments to develop individualized care plans.
- Provide skilled nursing services, including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor patient progress, identify changes in conditions, and communicate with care teams and physicians to ensure optimal outcomes.
- Educate patients and their families on disease management, medications, and preventive care techniques.
- Complete timely and accurate documentation using Home Care Home Base (HCHB).
- Collaborate with physical and occupational therapists, social workers, and home health aides for cohesive patient care.
- Participate in interdisciplinary case conferences, staff meetings, and continuing education to maintain high care standards.
- Adhere to regulatory requirements and agency protocols for documentation and patient safety.
Qualifications:
- Active RN license in the state of practice
- At least 1 year of recent home health field experience
- Skilled in using Home Care Home Base (HCHB) and completing OASIS assessments
- Strong clinical assessment, time management, and communication skills
- Reliable transportation, current drivers license, and valid auto insurance
- Comfortable working independently in a field-based environment
Preferred:
- BSN
- Prior experience managing caseloads of 2532 points per week
- Familiarity with Medicare and home health compliance standards
