Seeking an experienced and compassionate Registered Nurse to provide skilled care and support to patients in a home health setting. This rewarding position offers the opportunity to make a significant impact on patient outcomes by delivering patient-centered, coordinated care in a field-based role.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Assignment Details:
- Full-time, 13-week contract
- Field-based role visiting patient homes daily within the assigned territory (mileage reimbursement available)
- Expected caseload of 2532 points per week (approximately 35 patient visits per day)
- Documentation completed in Home Care Home Base (HCHB)
- Use of OASIS assessment tool
Key Responsibilities:
- Conduct comprehensive OASIS assessments and create tailored care plans for patients in their homes
- Deliver skilled nursing services including wound care, IV therapy, medication management, and chronic disease education
- Monitor patient progress, identify changes in condition, and collaborate closely with care teams and physicians
- Educate patients and families on health conditions, medications, safety, and preventive care
- Maintain detailed and timely documentation in accordance with regulatory standards
- Coordinate care with therapists, social workers, and home health aides
- Participate in case conferences, meetings, and continuing education sessions
Qualifications:
- Active RN license in good standing
- Minimum one year of recent field-based home health experience
- Proficiency with Home Care Home Base (HCHB) and use of OASIS
- Strong patient assessment and clinical skills
- Able to work independently and prioritize in a dynamic field environment
- Reliable transportation, valid drivers license, and current auto insurance
- Excellent organizational and interpersonal communication skills
Preferred:
- BSN degree
- Experience managing a caseload of 2532 points per week
- Knowledge of Medicare and home health compliance
