Deliver skilled, patient-focused care in the comfort of patients homes as a Home Health Registered Nurse. This contract opportunity is ideal for a motivated professional eager to make a meaningful impact through one-on-one support and clinical expertise. You will assess, plan, and deliver individualized care to patients with diverse needspromoting better health outcomes and supporting independence at home.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Field-based visits: 35 patient visits per day (2532 points per week)

Key Responsibilities:

• Complete thorough OASIS assessments and develop patient-specific care plans.
• Deliver a variety of skilled nursing services including wound care, IV therapy, chronic disease management, and medication administration.
• Monitor patients, update care plans, and communicate changes with the interdisciplinary team and physicians.
• Teach patients and families about conditions, treatments, safety, and preventive care to foster engagement and adherence.
• Maintain accurate, timely electronic documentation using Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides for coordinated patient support.
• Attend case conferences, contribute to staff meetings, and participate in ongoing education.

Qualifications:

• Active RN license (state of practice), in good standing.
• At least 1 year of recent home health (field) nursing experience.
• Experience with HCHB and OASIS required.
• Strong assessment, communication, and critical thinking abilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work autonomously and manage a dynamic schedule.
• BSN and previous experience managing 2532 points/week preferred.
• Familiarity with Medicare and home health regulations a plus.
