This field-based, full-time, 13-week contract position involves daily travel within your assigned territory, offering the flexibility and autonomy of home health nursing. As a Home Health RN, you'll provide high-quality, individualized care, leveraging your leadership and assessment skills while working independently. Collaborate with multidisciplinary teams, experience different care models, and expand your clinical portfolio with specialized home health best practices and innovative documentation techniques.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct OASIS assessments and develop tailored care plans for each patient.
• Deliver skilled nursing interventions, including wound care, IV therapy, and medication administration.
• Monitor patient progress, communicate changes to care teams and physicians promptly.
• Educate patients and families on diagnoses, treatments, and disease management.
• Complete accurate documentation within Home Care Home Base (HCHB).
• Coordinate care with PT, OT, social workers, and aides.
• Attend case conferences, staff meetings, and required continuing education.

Qualifications:

• Active registered nurse (RN) license in PA (in good standing).
• Minimum 1 year of recent field-based Home Health RN experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong clinical judgment, organization, and independent work capability.
• BSN and prior high-volume caseload management (preferred).
• Familiarity with Medicare and home health compliance is a plus.

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
