Job Description
Deliver compassionate, high-quality care as a skilled Home Health Registered Nurse supporting patients in their homes. This opportunity allows you to make a meaningful impact through direct patient care, individualized assessments, and collaboration with an interdisciplinary healthcare team.
Schedule & Assignment Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points per week (35 patient visits per day typical)
- Field-based position (travel within assigned territory; mileage reimbursement provided)
- Electronic documentation via Home Care Home Base (HCHB)
- OASIS assessment tool utilized
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing interventions (wound care, IV therapy, medication administration, chronic disease management)
- Monitor patient progress, communicating changes to healthcare team and physicians
- Educate patients and families about treatment plans, medications, and preventive care
- Maintain accurate, timely documentation in HCHB
- Collaborate with therapists, social workers, and home health aides for coordinated, patient-centered care
- Participate in case conferences, staff meetings, and ongoing education as necessary
- Adhere to regulatory and agency standards for documentation and patient care
Qualifications:
- Active RN License (in good standing) in state of practice
- At least 1 year of recent Home Health RN experience (field experience preferred)
- Proficient with Home Care Home Base (HCHB) and OASIS
- Demonstrated clinical assessment and critical thinking abilities
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work autonomously and manage a flexible, mobile schedule
- Excellent communication, organizational, and time management skills
- BSN preferred; prior caseload management of 2532 points per week is a plus
- Familiarity with Medicare and home health regulations beneficial
