Provide skilled nursing care to patients in their homes as a Home Health Registered Nurse (RN) in Independence, MO. This full-time, 13-week contract position involves delivering patient-centered care focused on quality and continuity. The role requires strong clinical skills, self-motivation, and the ability to work independently within a structured schedule.

Schedule/Hours:

• Monday Friday
• 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify condition changes, and communicate promptly with care teams and physicians.
• Provide education to patients and families on disease management, medications, and preventive care.
• Accurately document all care and assessments in Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated care.
• Participate in case conferences, staff meetings, and continuing education.
• Comply with regulatory and agency standards for patient care and documentation.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum one year recent home health experience (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS.
• Strong clinical assessment and critical thinking abilities.
• Reliable transportation, valid drivers license, and insurance.
• Excellent communication, time management, and organizational skills.
• Ability to work independently and manage a flexible field-based schedule.

Preferred:

• BSN degree.
• Experience managing 2532 points weekly.
• Knowledge of Medicare and home health compliance standards.
