An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care to patients in their homes, ensuring quality and patient-centered outcomes. This full-time, 13-week contract role offers a dynamic field-based environment where clinical expertise and independent working skills are essential. The schedule is Monday through Friday, from 8:00 AM to 5:00 PM, with a caseload expectation of 2532 points weekly, generally 35 patient visits per day.

Responsibilities include:

• Conducting comprehensive OASIS assessments and developing individualized care plans.
• Providing skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitoring patient progress, identifying condition changes, and communicating effectively with care teams and physicians.
• Educating patients and their families on treatment plans, medications, and preventive care.
• Maintaining accurate and timely documentation using the Home Care Home Base (HCHB) electronic system.
• Collaborating with interdisciplinary teams including therapists, social workers, and home health aides.
• Participating in case conferences, staff meetings, and ongoing education.
• Ensuring compliance with regulatory and agency standards.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum of 1 year recent Home Health experience with field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical assessment, critical thinking, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently managing a flexible field schedule.

Preferred:

• BSN degree.
• Previous experience managing 2532 points per week.
• Familiarity with Medicare and home health compliance standards.
