A skilled and compassionate Registered Nurse with home health experience is needed for a contract position delivering patient-centered, high-quality care directly in clients' homes. This full-time, 13-week assignment is ideal for professionals who thrive working independently and are confident in their clinical abilities and communication. Organization, adaptability, and a focus on clinical excellence are essential.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop personalized patient care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication management, and chronic disease education.
• Monitor patient progress, recognize changes in condition, and coordinate with physicians and healthcare team members.
• Provide patient and family education on disease processes, medication management, and preventive care.
• Accurately document all patient care in Home Care Home Base (HCHB).
• Work closely with physical therapists, occupational therapists, social workers, and home health aides to ensure coordinated care delivery.
• Attend regular case conferences and participate in continuing education.

Qualifications:

• Active RN license in good standing within the state.
• At least 1 year of recent field-based home health experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation.
• Exceptional clinical assessment, critical thinking, and organizational skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to efficiently manage caseloads of 2532 points per week (35 visits daily).
• Strong interpersonal and communication abilities.
• BSN and Medicare/home health compliance knowledge preferred.

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
