An opportunity is available for a skilled and compassionate Home Health Registered Nurse (RN) to provide high-quality nursing care to patients in their homes. This role centers on delivering patient-focused care, supporting recovery and independence, and ensuring each patient's needs are met with professionalism and empathy.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload expectation: 2532 points per week (typically 35 visits per day)
• Daily field-based travel to patient homes (mileage reimbursement provided)

Responsibilities:

• Deliver comprehensive OASIS assessments and design individualized care plans
• Administer skilled nursing interventions, including wound care, IV therapy, medication management, and chronic disease oversight
• Monitor patient progress, communicate changes promptly to physicians and the care team
• Educate patients and families regarding care plans, treatments, medications, and preventive health measures
• Maintain accurate and timely documentation within Home Care Home Base (HCHB)
• Collaborate with interdisciplinary teams, including physical and occupational therapists, medical social workers, and home health aides
• Participate in staff meetings, case conferences, and continuing education initiatives
• Ensure compliance with all regulatory and agency care delivery standards

Qualifications:

• Active, unencumbered RN license in the state of practice
• Minimum 1 year recent home health (field) RN experience
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Strong clinical judgment, assessment, and communication skills
• Reliable transportation, valid drivers license, and current auto insurance
• BSN and experience with managing 2532 points per week preferred
• Familiarity with Medicare and home health compliance a plus

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
