Providing skilled nursing care directly in patients homes, this position requires a dedicated and experienced Home Health Registered Nurse (RN) to deliver high-quality, patient-centered care. The role emphasizes comprehensive assessments, individualized care planning, and collaboration with multidisciplinary teams to achieve optimal health outcomes.

Location: Hyattsville, MD
Job Type: Full-time Contract (13 weeks)
Schedule:

• Monday Friday
• 8:00 AM 5:00 PM

Responsibilities:

• Conduct thorough OASIS assessments and develop personalized care plans.
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, detect changes in condition, and communicate promptly with healthcare providers.
• Educate patients and families on health conditions, treatments, and preventive strategies.
• Maintain accurate documentation using Home Care Home Base (HCHB) software.
• Coordinate care with physical therapists, occupational therapists, social workers, and home health aides.
• Participate in case conferences, staff meetings, and ongoing education.
• Comply with regulatory and agency standards related to patient care and documentation.

Qualifications:

• Active RN license in good standing in the state of practice.
• Minimum of 1 year recent Home Health nursing experience, preferably field-based.
• Proficiency with Home Care Home Base (HCHB) and OASIS tools is required.
• Strong clinical assessment and critical thinking skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently and manage a flexible schedule.
• Excellent communication, organization, and time management abilities.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing caseloads of 2532 points weekly.
• Knowledge of Medicare and home health compliance standards.
