Experienced and compassionate nursing professionals excel in this dynamic Home Health Registered Nurse (RN) opportunity, where patient-focused clinical care is delivered directly in client homes. This contract position empowers RNs to manage and provide holistic care while building relationships that support recovery, independence, and quality of life.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Field-based with daily travel to patient homes
- Caseload: 2532 points per week (typically 35 patient visits daily)
Required Qualifications:
- Active RN license in good standing
- At least 1 year of recent Home Health nursing experience (field-based strongly preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Reliable transportation with valid drivers license and auto insurance
- Excellent communication, organizational, and time management skills
- Ability to work independently, demonstrating strong clinical assessment and critical thinking
Preferred Qualifications:
- BSN degree
- Experience managing a caseload of 2532 points per week
- Understanding of Medicare and home health compliance requirements
Core Responsibilities:
- Conduct detailed OASIS assessments and create individualized care plans
- Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management
- Observe and document patient progress, responding promptly to changes in condition
- Educate patients and families on care plans, medication, and health maintenance
- Maintain thorough and timely clinical documentation using HCHB
- Collaborate with interdisciplinary care teams to ensure comprehensive care delivery
- Participate in case conferences, meetings, and ongoing professional development
- Maintain compliance with regulatory and agency standards
