Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care directly in patients homes. This full-time, 13-week contract opportunity allows you to provide quality, patient-centered care while working independently within a structured and supportive environment. You will perform comprehensive assessments, develop individualized care plans, and coordinate with interdisciplinary teams to ensure optimal outcomes for patients managing chronic conditions and post-acute needs.

Key Responsibilities:

  • Conduct thorough OASIS assessments and develop personalized care plans.
  • Administer skilled nursing care including wound care, medication management, IV therapy, and chronic disease support.
  • Monitor patient progress, identify changes in condition, and communicate updates to the care team and physicians promptly.
  • Educate patients and families about disease management, medications, and preventive health practices.
  • Maintain accurate documentation using Home Care Home Base (HCHB).
  • Collaborate effectively with therapists, social workers, and home health aides to coordinate comprehensive care.
  • Participate in case conferences, staff meetings, and ongoing professional development.
  • Adhere to all regulatory standards and agency policies for patient care and documentation.

Qualifications and Desired Experience:

  • Active RN license in good standing within the state of practice.
  • Minimum one year recent home health nursing experience; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
  • Strong critical thinking and clinical assessment abilities.
  • Reliable transportation, valid drivers license, and auto insurance for daily travel.
  • Excellent organizational, communication, and time management skills.
  • BSN preferred.
  • Experience managing 2532 points weekly and knowledge of Medicare home health regulations highly desirable.

Schedule and Location:

  • Monday through Friday, 8:00 AM to 5:00 PM.
  • Field-based role requiring daily travel to patient homes within an assigned territory.
  • Mileage reimbursement provided.
