An experienced and compassionate Home Health Registered Nurse (RN) is needed to deliver skilled nursing care directly in patients homes. This full-time, 13-week contract opportunity allows you to provide quality, patient-centered care while working independently within a structured and supportive environment. You will perform comprehensive assessments, develop individualized care plans, and coordinate with interdisciplinary teams to ensure optimal outcomes for patients managing chronic conditions and post-acute needs.
Key Responsibilities:
- Conduct thorough OASIS assessments and develop personalized care plans.
- Administer skilled nursing care including wound care, medication management, IV therapy, and chronic disease support.
- Monitor patient progress, identify changes in condition, and communicate updates to the care team and physicians promptly.
- Educate patients and families about disease management, medications, and preventive health practices.
- Maintain accurate documentation using Home Care Home Base (HCHB).
- Collaborate effectively with therapists, social workers, and home health aides to coordinate comprehensive care.
- Participate in case conferences, staff meetings, and ongoing professional development.
- Adhere to all regulatory standards and agency policies for patient care and documentation.
Qualifications and Desired Experience:
- Active RN license in good standing within the state of practice.
- Minimum one year recent home health nursing experience; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
- Strong critical thinking and clinical assessment abilities.
- Reliable transportation, valid drivers license, and auto insurance for daily travel.
- Excellent organizational, communication, and time management skills.
- BSN preferred.
- Experience managing 2532 points weekly and knowledge of Medicare home health regulations highly desirable.
Schedule and Location:
- Monday through Friday, 8:00 AM to 5:00 PM.
- Field-based role requiring daily travel to patient homes within an assigned territory.
- Mileage reimbursement provided.
