A skilled and compassionate nursing professional is sought to provide outstanding patient-centered care in a home health setting. This role offers a dynamic and rewarding environment for an experienced RN who excels at delivering high-quality clinical care independently, while maintaining strong organizational and communication abilities.

Schedule:

• Full-time, 13-week contract
• Monday through Friday, 8:00 AM 5:00 PM
• Daily travel to patient homes within an assigned territory

Key Duties:

• Complete comprehensive OASIS assessments and create individualized care plans for homebound patients.
• Deliver skilled nursing interventions, including wound care, medication management, IV administration, and chronic disease oversight.
• Closely monitor patient conditions, update care plans as needed, and communicate promptly with healthcare providers.
• Provide thorough patient and family education on disease processes, medication protocols, and preventative healthcare.
• Maintain accurate, timely records using Home Care Home Base (HCHB) and ensure compliance with regulatory and agency standards.
• Coordinate care with an interdisciplinary team, participating in collaborative meetings and ongoing education.

Qualifications:

• Active, unencumbered RN license in the state of practice
• At least one year of recent clinical experience in home health, with field experience preferred
• Proficiency in Home Care Home Base (HCHB) and OASIS
• Strong clinical assessment, critical thinking, and patient education skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to independently structure a field-based workweek and manage a flexible caseload
• BSN and prior caseload management experience (2532 points per week) are preferred
• Knowledge of Medicare and home health compliance is a plus

$1,709 - $1,807/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
