Job Description
An experienced Home Health Registered Nurse (RN) is needed to deliver skilled nursing care to patients in home settings within Horse Cave, KY. This full-time, 13-week contract position requires a self-motivated nurse comfortable working independently, managing complex cases, and communicating effectively with patients and interdisciplinary teams.
Responsibilities include performing comprehensive OASIS assessments, developing individualized care plans, administering treatments such as wound care, medication, and IV therapy, and educating patients and families on care and prevention. The RN will evaluate patient progress, document thoroughly in Home Care Home Base (HCHB), and collaborate with therapists, social workers, and aides to ensure seamless care delivery within a field-based setting. Adherence to regulatory standards and participation in staff meetings and continuing education are essential.
Qualifications:
- Active RN license in good standing for the state of practice
- Minimum of 1 year recent home health experience, preferably field-based
- Proficient in Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment, critical thinking, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently and manage a flexible schedule
Preferred:
- BSN degree
- Experience managing 2532 patient care points per week
- Knowledge of Medicare and home health compliance
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Approximately 35 patient visits per day with daily travel within assigned territory (mileage reimbursement provided)
