An exciting opportunity is available for an experienced and compassionate nurse to deliver high-quality patient care in a home health setting. This contract role is ideal for a registered nurse who is organized, self-motivated, and dedicated to clinical excellence and effective patient communication.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

What You'll Do:

• Conduct thorough OASIS assessments and create individualized care plans that target positive outcomes.
• Deliver a full spectrum of skilled nursing care in patients' homes, including wound care, medication administration, IV therapy, and management of chronic conditions.
• Monitor patient progress, address changes in condition, and maintain open, timely communication with physicians and the care team.
• Educate patients and families on diagnosis, treatment plans, medications, and preventive health practices.
• Maintain detailed and accurate electronic documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary professionalsincluding therapists, social workers, and home health aidesto ensure coordinated, quality care.
• Regularly participate in staff meetings, case conferences, and continuing education.
• Uphold all regulatory and organizational standards in documentation and care delivery.
• Mileage reimbursement provided for daily travel within the assigned territory.

Requirements:

• Active RN license in good standing.
• At least 1 year of recent field-based Home Health experience.
• Proficiency in Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment, organizational, and critical thinking abilities.
• Reliable vehicle, valid drivers license, and auto insurance.
• Comfort with independent, flexible, field-based work.
• Excellent communication and time management.

Preferred:

• BSN degree
• Experience managing a caseload of 2532 points per week.
• Knowledge of Medicare and home health compliance.

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
