Deliver high-quality, compassionate patient care in the comfort of each home as a skilled Registered Nurse in a home health field-based role. Work closely with patients, families, and interdisciplinary healthcare teams to achieve optimal outcomes while maintaining autonomy over your daily schedule. We are seeking an experienced RN with a strong track record in home health, OASIS assessments, and electronic medical records to support a diverse patient caseload.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized plan of care
- Deliver hands-on skilled nursing including wound care, medication administration, IV therapy, and chronic disease management
- Monitor and evaluate patient progress, updating the care plan and communicating changes to the broader healthcare team
- Educate patients and caregivers on conditions, medications, treatment plans, and promote health maintenance
- Accurately document all assessments, visits, and patient interactions in Home Care Home Base (HCHB)
- Collaborate with therapists, social workers, and home health aides to coordinate holistic care
- Participate in interdisciplinary case conferences and ongoing professional development
- Adhere to regulatory and agency patient care standards
Required Qualifications:
- Active RN license (state of practice, in good standing)
- Minimum 1 year of recent home health experience (field-based preferred)
- Strong proficiency in Home Care Home Base (HCHB) and OASIS documentation
- Reliable transportation, valid driver's license, and auto insurance
- Demonstrated self-motivation, time management, and organizational skills
Preferred Qualifications:
- BSN degree
- Experience managing 2532 points per week
- Knowledge of Medicare and home health compliance
