This full-time, 13-week contract role is ideal for a Home Health Registered Nurse who enjoys autonomy, flexibility, and making an impact in community-based care. Youll play a crucial part in helping patients achieve the best possible outcomes, supported by modern technology and a collaborative team.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Qualifications:
- Active RN license (in good standing) in the state of practice
- Minimum of one year of recent home health experience (field experience strongly preferred)
- Proficiency in Home Care Home Base (HCHB) and use of OASIS assessment required
- Ability to manage a caseload of 2532 points per week (typically 35 visits daily)
- Strong clinical assessment, communication, and critical thinking abilities
- Reliable transportation with valid drivers license and current auto insurance
- BSN preferred and prior experience with Medicare home health compliance is a plus
Primary Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
- Provide patient and family education on disease processes, care planning, and medication management
- Closely monitor patient progress and coordinate interventions with the interdisciplinary team
- Ensure documentation meets regulatory and agency standards using Home Care Home Base
- Participate in case conferences, team meetings, and continuous professional development
