Join an established home health nursing team as a skilled Registered Nurse providing compassionate, patient-centered care in clients homes across the Hillsboro, OR area. This full-time, 13-week contract role offers a rewarding opportunity to combine clinical expertise with independent fieldwork, ensuring quality and continuity of care for diverse home health patients.
Key responsibilities include conducting thorough OASIS assessments and developing personalized care plans, delivering skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management, as well as educating patients and their families. Youll continuously evaluate patient progress, communicate condition changes to the interdisciplinary team and physicians, and accurately maintain documentation within the Home Care Home Base (HCHB) system. Collaboration with therapists, social workers, and aides will support holistic and coordinated care delivery.
Qualifications:
- Active RN license in good standing for Oregon
- Minimum one year of recent home health nursing experience, preferably field-based
- Proficient use of Home Care Home Base (HCHB) and OASIS assessment tools
- Strong clinical assessment abilities and critical thinking
- Reliable transportation with valid drivers license and auto insurance
- Self-motivated with excellent communication, time management, and organizational skills
Preferred:
- BSN degree
- Experience managing caseloads of 2532 points per week
- Familiarity with Medicare and home health regulatory standards
Schedule & Caseload:
- Monday through Friday, 8:00 AM to 5:00 PM
- 3-5 patient visits daily, averaging 2532 points weekly
- Daily travel within assigned territory with mileage reimbursement
