Embark on a rewarding opportunity as a Home Health Registered Nurse, delivering compassionate and skilled nursing care directly to patients in their homes. This 13-week full-time contract role offers you the chance to make a meaningful impact on patient well-being while working autonomously in a dynamic field setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (roughly 35 patient visits daily)

Key Qualifications:

• Current RN license in good standing
• At least 1 year of recent home health experience (field visits preferred)
• Expertise with OASIS assessments and documentation
• Proficiency in Home Care Home Base (HCHB) electronic system
• Strong clinical skills, assessment abilities, and problem-solving capacity
• Excellent organizational and communication skills
• Self-directed, reliable, with valid drivers license, insurance, and dependable vehicle

Preferred:

• BSN degree
• Experience managing 2532 points weekly
• Familiarity with Medicare and home health compliance

Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing - wound care, IV therapy, medication administration, chronic disease management
• Monitor patient status, promptly communicate condition changes, and ensure seamless collaboration with interdisciplinary teams
• Educate patients and families on treatment regimens and preventive care
• Maintain thorough, timely documentation in HCHB
• Travel daily within the assigned territory to patient homes (mileage reimbursement available)
• Participate in case conferences, staff meetings, and continuing education as needed
• Uphold regulatory standards and agency expectations for quality care

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
