Job Description

An opportunity is available for an experienced and compassionate Registered Nurse to deliver high-quality care in a home health setting. This position is ideal for clinicians who thrive in autonomous field-based roles and are dedicated to providing patient-centered nursing services.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct thorough OASIS assessments and develop individualized care plans for home health patients.
  • Perform skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, promptly identify changes in condition, and communicate effectively with care teams and physicians.
  • Educate patients and families regarding diagnoses, treatment plans, medications, and preventive health practices.
  • Accurately document all care and interventions in Home Care Home Base (HCHB).
  • Collaborate with physical therapists, occupational therapists, medical social workers, and aides for coordinated care delivery.
  • Participate in case conferences, staff meetings, and ongoing professional development.
  • Ensure all care and documentation meet regulatory and agency standards.

Qualifications:

  • Active RN license in good standing for the state of practice.
  • At least 1 year of recent experience in home health nursing (field-based strongly preferred).
  • Working knowledge of Home Care Home Base (HCHB) and OASIS required.
  • Demonstrated clinical assessment and critical thinking skills.
  • Reliable transportation, valid drivers license, and auto insurance required.
  • Strong communication, time management, and organizational skills.
  • BSN preferred.
  • Previous caseload management of 2532 points per week preferred.
  • Understanding of Medicare and home health compliance is a plus.
