An opportunity is available for a dedicated and experienced Registered Nurse with a passion for home health care. This role focuses on delivering skilled, patient-centered care to individuals within their own homes, supporting optimal health outcomes and enhancing quality of life. The contract spans 13 weeks, offering a structured schedule designed for professional growth and work-life balance.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Responsible for managing a caseload of 2532 points per week, typically 35 patient visits daily
• Daily field-based travel to patient residences within your assigned territory (mileage reimbursement included)

Key Responsibilities:

• Conduct thorough OASIS assessments and formulate individualized care plans
• Deliver high-quality nursing interventionswound care, medication administration, IV therapy, and chronic disease management
• Track patient progress and communicate updates or changes to the interdisciplinary care team and physicians
• Educate patients and their families on their health conditions, medications, and support preventive health practices
• Accurately document all care activities in the Home Care Home Base (HCHB) system
• Collaborate with therapists, social workers, and aides for cohesive patient care
• Participate actively in staff meetings, case conferences, and required training

Desired Qualifications:

• Active RN license in good standing
• At least 1 year of recent field-based home health RN experience
• Comfort and proficiency with HCHB and OASIS required
• Strong communication, assessment, and time-management skills
• Reliable transportation, valid driver's license, and auto insurance
• BSN and prior experience managing similar caseloads preferred
• Knowledge of Medicare and home health standards advantageous

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
