Take on a rewarding opportunity providing essential care in a field-based setting as a Home Health Registered Nurse. In this contract role, youll deliver high-quality nursing services directly to patients in their homes, making a vital difference in their recovery and quality of life. The ideal candidate is dependable, self-driven, and adept at delivering clinical excellence with compassionate communication.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Typical caseload: 2532 points per week (approximately 35 patient visits daily)
- Daily travel within an assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Complete comprehensive OASIS assessments and create personalized care plans.
- Deliver skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, identify changes in condition, and communicate effectively with care teams and physicians.
- Educate patients and families on disease processes, medications, treatment protocols, and prevention.
- Maintain thorough and timely documentation using Home Care Home Base (HCHB).
- Partner with a multidisciplinary team, including therapists and social workers, to coordinate care.
- Participate in case conferences, meetings, and ongoing professional development.
- Ensure adherence to all regulatory and best practice standards in home health.
Qualifications:
- Active RN license (in good standing) in the state of practice
- At least 1 year of recent, relevant Home Health field experience
- Proficient with HCHB and OASIS documentation
- Effective assessment, time management, and organizational abilities
- Valid drivers license, reliable transportation, and auto insurance
- BSN and prior high-volume caseload experience preferred
