Job Description

Deliver essential care where it matters mostright in patients homes. As a Home Health Registered Nurse, youll use your clinical skills and compassion to make a meaningful impact, providing skilled nursing care and guidance to individuals throughout their recovery journeys.

Schedule/Hours:

• Full-time 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Daily caseload expectation: 2532 points per week (typically 35 patient visits per day)
• Field-based, with compensated daily travel within your assigned territory

Key Responsibilities:

• Perform comprehensive patient assessments using OASIS and develop individualized care plans
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management
• Track patient progress; communicate changes promptly to physicians and interdisciplinary care teams
• Educate patients and families about diagnoses, treatment plans, disease processes, and preventive strategies
• Document visits and care precisely and efficiently in the Home Care Home Base (HCHB) platform
• Coordinate care with physical therapists, occupational therapists, social workers, and home health aides
• Participate in case conferences, staff meetings, and professional development as required
• Ensure adherence to all regulatory and agency documentation standards

Qualifications:

• Active RN license in good standing
• At least 1 year of recent home health field experience
• Proficiency in Home Care Home Base (HCHB) and OASIS documentation
• Strong clinical assessment and organizational abilities
• Reliable transportation, valid drivers license, and current auto insurance
• BSN and prior success managing 25-32 point caseloads (preferred)
• Excellent interpersonal and communication skills
