Deliver skilled care and make a meaningful impact in patients lives as an experienced Home Health Registered Nurse. In this dynamic, field-based role, you will use your clinical expertise and compassion to ensure high-quality, patient-centered care for individuals in their homes. Ideal candidates are self-driven, highly organized, and thrive working independently, maintaining exceptional standards in both care delivery and communication.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
- 13-week full-time contract
- Caseload of 2532 points weekly (typically 35 daily home visits)
- Field-based within an assigned territory, with daily travel and mileage reimbursement
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop tailored care plans
- Deliver skilled nursing care: wound care, IV therapy, medication administration, and disease management
- Monitor and evaluate patients, identifying changes in condition and communicating with the interdisciplinary team
- Educate patients and families about disease processes, management, and preventive strategies
- Maintain precise and timely documentation within Home Care Home Base (HCHB)
- Coordinate with therapists, social workers, and aides to ensure seamless patient care
- Participate in case conferences, staff meetings, and ongoing education
- Uphold regulatory and agency care standards
Qualifications:
- Current, active RN license (in good standing)
- Minimum one year recent home health experience; field experience strongly preferred
- Proficiency in Home Care Home Base (HCHB) and OASIS required
- Strong clinical assessment, communication, and time-management skills
- Reliable transportation, valid drivers license, and required auto insurance
- BSN preferred; prior experience with 2532 points per week is a plus
- Knowledge of Medicare/home health compliance standards preferred
