Providing skilled nursing care in patients homes, this role requires a compassionate and experienced Home Health Registered Nurse (RN) focused on quality, continuity, and patient-centered care. The ideal candidate is self-motivated, organized, and comfortable working independently while upholding high standards of clinical excellence and communication.

Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes, and communicate promptly with care teams and physicians.
• Educate patients and families on disease processes, treatments, medications, and preventive care.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members such as physical therapists, occupational therapists, medical social workers, and home health aides.
• Participate in case conferences, staff meetings, and continuing education.
• Adhere to regulatory and agency standards for documentation and care delivery.

Qualifications:

• Active RN license in good standing in the state of practice.
• Minimum of 1 year recent home health experience; field experience preferred.
• Proficiency in Home Care Home Base (HCHB) and OASIS experience required.
• Strong clinical assessment and critical thinking abilities.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to work independently and manage a flexible, field-based schedule.
• Excellent communication, time management, and organizational skills.

Preferred:

• BSN degree.
• Experience managing 2532 points/week caseload.
• Knowledge of Medicare and home health compliance standards.

Job Details:

• Location: Hawthorne, NY
• Contract Type: Full-time, 13-week contract
• Schedule: Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits daily)
• Field-based with mileage reimbursement provided
