Job Description
Deliver skilled and compassionate nursing care directly to patients in their homes, ensuring each individual receives patient-centered, continuous, and high-quality services. This contract position is ideal for an experienced Home Health RN who thrives in an independent, field-based work environment and values direct patient interaction and comprehensive care management.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Responsibilities:
- Complete OASIS assessments and develop individualized care plans for home health patients.
- Deliver skilled nursing including wound care, IV therapy, chronic illness management, and medication administration in patients homes.
- Monitor patient status, identify and address changes, and coordinate interventions in collaboration with physicians and the healthcare team.
- Provide education and support to patients and families about diagnoses, treatment plans, medication regimens, and preventive health.
- Document all care and assessments using Home Care Home Base (HCHB) in a timely and accurate manner.
- Collaborate with PT/OT, medical social workers, and aides to ensure holistic, compliant, and effective care.
- Attend case conferences, meetings, and complete continuing education as necessary.
- Maintain compliance with all documentation and care delivery standards for regulatory and agency requirements.
Qualifications:
- Active RN license in good standing.
- At least 1 year of recent home health nursing experience (field preferred).
- Experience with Home Care Home Base (HCHB) and OASIS required.
- Strong clinical, assessment, and communication skills.
- Ability to work efficiently and independently, managing a caseload of 2532 points per week.
- Reliable vehicle, valid drivers license, and current auto insurance.
- BSN and knowledge of home health/Medicare documentation preferred.
