Experienced and compassionate nurses make a meaningful difference every day as Home Health Registered Nurses. In this contract role, youll deliver skilled, patient-centered care to clients in their homes, fostering independence and supporting optimal outcomes in a rewarding field-based setting.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Details:

• Caseload: 2532 points per week (typically 35 patient visits daily)
• Assessment Tool: OASIS
• Documentation: Home Care Home Base (HCHB)
• Travel: Daily to patient residences within assigned territory (mileage reimbursement provided)

Responsibilities:

• Conduct comprehensive OASIS assessments and establish individualized care plans.
• Provide in-home skilled nursing, including medication administration, wound care, IV therapy, and chronic disease management.
• Evaluate patient progress and report changes to the interdisciplinary care team and physicians.
• Deliver education to patients and families about disease processes, medications, and treatment options.
• Maintain accurate documentation using Home Care Home Base (HCHB).
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides for coordinated patient care.
• Participate in staff meetings, case conferences, and ongoing education.
• Uphold regulatory and agency standards in all aspects of care and recordkeeping.

Qualifications:

• Active RN license (in good standing)
• At least one year of recent home health experience (field experience preferred)
• Proficiency in Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, valid drivers license, and current auto insurance
• Strong clinical, critical thinking, and organizational skills
• BSN and knowledge of Medicare/home health compliance standards are preferred
