Job Description

An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide skilled nursing care to patients in their homes. This full-time, 13-week contract position offers the opportunity to deliver patient-centered care, ensuring quality and continuity while working independently in a field-based setting.

Location:
Based in Haverhill, MA, this role involves travel within the assigned territory for patient home visits. Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identifying changes and communicating promptly with care teams and physicians.
  • Educate patients and families on conditions, treatments, medications, and preventive measures.
  • Maintain accurate, timely documentation in Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary teams including physical therapists, occupational therapists, social workers, and home health aides.
  • Participate in case conferences, staff meetings, and ongoing education.
  • Adhere to regulatory and agency standards for care delivery and documentation.

Qualifications / Desired Experience:

  • Active RN license in good standing for the state of practice.
  • Minimum 1 year of recent home health experience (field experience preferred).
  • Proficiency in Home Care Home Base (HCHB) and OASIS assessment tools required.
  • Strong clinical assessment, critical thinking, communication, and organizational skills.
  • Reliable transportation with valid drivers license and auto insurance.
  • Ability to work independently and manage a flexible field-based schedule.
  • BSN preferred.
  • Experience managing 2532 points weekly is a plus.
  • Knowledge of Medicare and home health compliance standards.
