Job Description

Deliver skilled nursing care in patients homes as a Home Health Registered Nurse (RN), dedicated to promoting health and independence. This full-time, 13-week contract role requires a compassionate and organized professional comfortable managing a caseload of 2532 points weekly, typically involving 35 patient visits per day. Utilizing electronic documentation through Home Care Home Base (HCHB) and performing OASIS assessments, you will coordinate with interdisciplinary teams to create and implement individualized care plans.

Key Responsibilities:

  • Conduct thorough OASIS assessments and develop patient-centered care plans.
  • Provide skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress and communicate changes promptly with physicians and care teams.
  • Educate patients and families about health conditions, treatments, and prevention strategies.
  • Maintain accurate, timely documentation in HCHB.
  • Collaborate with therapists, social workers, and home health aides to ensure cohesive care delivery.
  • Participate in case conferences, staff meetings, and continuing education.
  • Comply with all regulatory and agency standards for documentation and patient care.

Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum one year recent home health nursing experience; field experience preferred.
  • Proficiency in Home Care Home Base (HCHB) and OASIS assessments.
  • Strong clinical assessment and critical thinking ability.
  • Reliable transportation, valid drivers license, and auto insurance.
  • Ability to work independently and manage a flexible field-based schedule.
  • Excellent communication, time management, and organizational skills.

Preferred:

  • Bachelor of Science in Nursing (BSN).
  • Experience managing 2532 points weekly.
  • Knowledge of Medicare and home health compliance standards.
