This field-based, full-time opportunity spans a 13-week contract and offers work-life balance with a Monday through Friday, 8:00 AM 5:00 PM schedule. Youll typically manage 35 patient visits per day (2532 points weekly), focusing on individualized care throughout your assigned territory.

Key Responsibilities:

• Perform comprehensive OASIS assessments and build personalized care plans for each patient.
• Deliver skilled nursing services such as wound care, medication management, IV therapy, and chronic disease oversight.
• Monitor patient conditions and adjust care as needed, ensuring ongoing communication with physicians and interdisciplinary teams.
• Educate patients and families about diagnosis, medications, treatment plans, and preventive strategies.
• Accurately document all interventions and updates in the Home Care Home Base (HCHB) system.
• Collaborate effectively with therapists, social workers, and home health aides for integrated care.
• Maintain compliance with regulatory and agency standards and participate in team meetings and educational sessions as needed.

Qualifications:

• Active, unencumbered RN license in the state of practice.
• At least 1 year of recent field Home Health RN experience.
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS.
• Strong critical thinking, organizational, and time management skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to work independently in a flexible, field-based environment. Preferred: BSN degree, experience managing a full caseload of 2532 points, and familiarity with Medicare and compliance requirements.

$1,709 - $1,807/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
