Deliver high-quality patient care as a Home Health Registered Nurse, providing skilled nursing services directly in patients homes for a 13-week contract assignment. Take on a critical role assessing patient needs, delivering individualized care, and ensuring coordination with an interdisciplinary medical team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and document patient progress; identify changes in patient condition and update the care team and physicians accordingly.
• Educate patients and families on disease processes, medications, and preventive strategies.
• Record all services and assessments using the Home Care Home Base (HCHB) electronic documentation system.
• Work closely with therapists, social workers, and aides, ensuring coordinated and holistic patient care.
• Participate in team case conferences, staff meetings, and pursue ongoing education as needed.
• Comply with all regulatory and documentation standards.

Qualifications:

• Active, unrestricted RN license in the state of practice.
• A minimum of one year of recent field-based Home Health RN experience.
• Proficiency using Home Care Home Base (HCHB) required.
• OASIS assessment experience required.
• Strong assessment, clinical, and critical thinking skills.
• Excellent communication, organizational, and time management abilities.
• Dependable transportation, valid drivers license, and auto insurance required.
• Ability to manage a field-based caseload of 2532 points per week.
• BSN preferred and knowledge of Medicare/home health compliance standards is a plus.
