Make a direct impact in the lives of patients as an experienced Home Health Registered Nurse (RN). This full-time, 13-week contract role offers the opportunity to deliver compassionate, individualized care in a field-based setting, serving patients in their homes and helping them achieve optimal outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in good standing
• Minimum of 1 year of recent home health nursing experience (field experience preferred)
• Demonstrated proficiency with Home Care Home Base (HCHB) electronic documentation
• OASIS assessment experience is required
• Strong clinical assessment, critical thinking, and organizational skills
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to work independently while managing a field-based, flexible schedule
• Excellent communication and time management abilities

Preferred:

• BSN credential
• Prior experience with a caseload of 2532 points per week
• Knowledge of Medicare/home health compliance

Primary Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care such as wound management, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, recognize changes in condition, and communicate with care teams and physicians
• Provide education to patients and caretakers on treatment plans, medications, and preventative health
• Maintain accurate, timely documentation in Home Care Home Base (HCHB)
• Collaborate with interdisciplinary professionals to ensure coordinated patient care
• Participate in case conferences, staff meetings, and ongoing education

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
