Job Description

We are looking for a dedicated and skilled Registered Nurse with home health experience to deliver high-quality care for patients in their homes. This role focuses on providing comprehensive, individualized nursing services while ensuring effective communication and collaboration with interdisciplinary teams.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete thorough OASIS assessments and develop patient-centered care plans.
• Perform skilled nursing duties such as wound care, medication administration, IV therapy, and chronic disease management.
• Assess patient progress, recognize changes in condition, and communicate with physicians and healthcare team members.
• Educate patients and families on diagnosis, medications, and health management strategies.
• Accurately document all care provided using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for comprehensive care coordination.
• Participate in case conferences, staff meetings, and ongoing education.
• Ensure compliance with regulatory and agency standards for patient care and documentation.
• Travel daily to patients homes within an assigned territory; mileage reimbursement provided.

Qualifications:

• Active and unrestricted RN license in Connecticut.
• At least 1 year of recent home health nursing experience, with field experience preferred.
• Proficient in Home Care Home Base (HCHB) and OASIS assessments.
• Strong assessment and critical thinking abilities.
• Reliable transportation, valid drivers license, and current auto insurance.
• Capacity to work autonomously and balance a flexible field-based schedule.
• Excellent interpersonal, time management, and organizational skills.

Preferred:

• BSN degree.
• Experience managing 2532 points per week in home health.
• Understanding of Medicare and home health compliance.
