An opportunity is available for a skilled and compassionate Home Health Registered Nurse to provide exceptional patient care in a field-based role. This contract assignment offers the chance to make a meaningful impact by delivering high-quality nursing care directly to individuals in their homes, supporting recovery and independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct OASIS assessments and design personalized care plans.
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize condition changes, and communicate updates to the interdisciplinary team and physicians.
• Educate patients and families regarding diagnoses, treatment strategies, medications, and prevention.
• Ensure timely and accurate documentation using Home Care Home Base (HCHB).
• Coordinate with physical and occupational therapists, medical social workers, and home health aides to ensure seamless care.
• Engage in case conferences, staff meetings, and ongoing education as needed.
• Uphold agency and regulatory requirements for documentation and patient care.

Qualifications:

• Current RN license (in good standing) for state of practice.
• At least 1 year of recent home health nursing experience required.
• Experience with Home Care Home Base (HCHB) and OASIS required.
• Reliable transportation, a valid driver's license, and auto insurance.
• Ability to work independently and efficiently in a field-based setting.
• Strong clinical assessment, communication, and organizational skills.
• BSN and familiarity with Medicare/home health compliance preferred.
• Proven experience managing caseloads of 2532 points per week is a plus.
