Seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract providing high-quality, patient-centered care in a field-based setting. This opportunity is ideal for a compassionate nurse who values independent work, flexible scheduling, and meaningful patient relationships.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (35 patient visits per day)
• Daily travel to patient homes within the assigned territory (mileage reimbursement provided)

Essential Responsibilities:

• Conduct comprehensive OASIS assessments and develop tailored care plans.
• Administer skilled nursing care such as wound care, IV therapy, chronic disease management, and medication administration.
• Monitor and document patient progress using Home Care Home Base (HCHB).
• Provide patient and family education on health management, medications, and preventive care.
• Collaborate with interdisciplinary teamsincluding physical/occupational therapists, social workers, and home health aidesfor coordinated, holistic care.
• Participate in case conferences, staff meetings, and ongoing education.
• Maintain regulatory compliance and uphold the highest standards for documentation and care.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum 1 year of recent field Home Health RN experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Strong clinical assessment, time management, and organizational skills.
• Valid drivers license, reliable transportation, and current auto insurance.
• Ability to work autonomously and manage a dynamic field schedule.
• BSN and prior caseload management of 2532 points per week preferred.
• Knowledge of Medicare and home health compliance is a plus.
