Job Description

A highly skilled and compassionate Registered Nurse is needed to deliver home health care services to patients in their residences. This full-time, 13-week contract position centers on supporting individuals in the comfort of their homes, ensuring patient safety, promoting recovery, and fostering independence. This opportunity is ideal for an organized, self-driven individual who thrives working autonomously and excels at building trust with patients and their families.

Schedule

  • Monday Friday, 8:00 AM 5:00 PM
  • Daily travel within assigned territory (mileage reimbursement provided)
  • Caseload: approximately 2532 points per week (typically 35 patient visits per day)

Responsibilities

  • Complete comprehensive OASIS assessments and develop individualized care plans
  • Provide skilled nursingwound care, IV therapy, medication administration, and chronic disease management
  • Evaluate patient progress, adjust care plans as needed, and promptly communicate changes to the interdisciplinary care team and physicians
  • Educate patients and families about disease and medication management, treatment options, and preventive health
  • Maintain accurate, timely documentation in Home Care Home Base (HCHB)
  • Collaborate with therapists, medical social workers, and home health aides for coordinated, holistic care
  • Attend case conferences, staff meetings, and participate in continuing education as appropriate
  • Uphold all regulatory and agency requirements for patient care and documentation

Qualifications

  • Active RN license (in good standing) for the state of practice
  • Minimum of one year of recent home health experience (field experience strongly preferred)
  • Proficient in Home Care Home Base (HCHB) and OASIS assessments
  • Reliable transportation, current driver's license, and automobile insurance
  • Strong clinical assessment, critical thinking, and communication skills
  • Able to work independently and manage a flexible schedule
  • BSN and experience managing a 2532-point weekly caseload preferred
  • Knowledge of Medicare and home health regulations desirable
