A highly skilled and compassionate Registered Nurse is needed to deliver home health care services to patients in their residences. This full-time, 13-week contract position centers on supporting individuals in the comfort of their homes, ensuring patient safety, promoting recovery, and fostering independence. This opportunity is ideal for an organized, self-driven individual who thrives working autonomously and excels at building trust with patients and their families.
Schedule
- Monday Friday, 8:00 AM 5:00 PM
- Daily travel within assigned territory (mileage reimbursement provided)
- Caseload: approximately 2532 points per week (typically 35 patient visits per day)
Responsibilities
- Complete comprehensive OASIS assessments and develop individualized care plans
- Provide skilled nursingwound care, IV therapy, medication administration, and chronic disease management
- Evaluate patient progress, adjust care plans as needed, and promptly communicate changes to the interdisciplinary care team and physicians
- Educate patients and families about disease and medication management, treatment options, and preventive health
- Maintain accurate, timely documentation in Home Care Home Base (HCHB)
- Collaborate with therapists, medical social workers, and home health aides for coordinated, holistic care
- Attend case conferences, staff meetings, and participate in continuing education as appropriate
- Uphold all regulatory and agency requirements for patient care and documentation
Qualifications
- Active RN license (in good standing) for the state of practice
- Minimum of one year of recent home health experience (field experience strongly preferred)
- Proficient in Home Care Home Base (HCHB) and OASIS assessments
- Reliable transportation, current driver's license, and automobile insurance
- Strong clinical assessment, critical thinking, and communication skills
- Able to work independently and manage a flexible schedule
- BSN and experience managing a 2532-point weekly caseload preferred
- Knowledge of Medicare and home health regulations desirable
