An opportunity is available for a skilled and dedicated Registered Nurse (RN) specializing in home health care to provide essential nursing services directly in patients homes. This full-time, 13-week contract position focuses on delivering compassionate, patient-centered care while ensuring clinical excellence and effective communication within an interdisciplinary healthcare team.
Location:
This position is based in Gresham, OR, a vibrant city known for its welcoming community atmosphere and proximity to outdoor recreational areas, including scenic parks and trails along the Columbia River Gorge. Gresham offers a mix of suburban comfort and accessibility to metropolitan amenities.
Schedule:
- Monday Friday
- 8:00 AM 5:00 PM
Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans tailored to patient needs.
- Provide skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, recognize changes in condition, and communicate effectively with physicians and care teams.
- Educate patients and families on health conditions, treatments, medications, and preventive care strategies.
- Maintain accurate and timely electronic health records using Home Care Home Base (HCHB).
- Collaborate closely with therapists, social workers, and home health aides to ensure holistic and coordinated care delivery.
- Participate in case conferences and continuing education to uphold best practices and compliance with regulatory standards.
Qualifications:
- Active RN license in good standing within the state of practice.
- Minimum of 1 year recent home health experience with field-based exposure preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
- Strong clinical judgment, critical thinking, and communication skills.
- Reliable transportation, valid drivers license, and auto insurance are required to travel within assigned territories.
- Ability to work independently and manage a flexible schedule.
Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience managing 2532 points per week caseload.
- Knowledge of Medicare and home health compliance standards.
