A rewarding opportunity is available for an experienced and compassionate Registered Nurse specializing in home health. In this contract-based role, youll deliver skilled, patient-focused nursing care in a home setting, ensuring quality, continuity, and a high standard of clinical excellence. The position is ideal for professionals who are self-motivated, organized, and can work independently while fostering strong communication with patients and the interdisciplinary care team.

Schedule & Details:

• MondayFriday, 8:00 AM5:00 PM
• Full-time, 13-week contract
• Caseload: 2532 points per week (generally 35 patient visits daily)
• Field-based, involving daily travel within your assigned territory

Key Responsibilities:

• Perform comprehensive OASIS assessments and craft individualized care plans
• Deliver skilled services: wound care, IV therapy, medication administration, chronic disease management
• Closely monitor patient progress, noting and communicating any changes in condition
• Educate patients and their families on disease processes, medications, and preventive health
• Maintain precise, timely records with Home Care Home Base (HCHB)
• Collaborate with physical/occupational therapists, social workers, and home health aides to ensure coordinated care
• Attend case conferences and staff meetings; engage in ongoing education
• Follow all agency and regulatory requirements for documentation and patient care

Qualifications:

• Active RN license (in good standing)
• At least 1 year of recent home health experience (field experience preferred)
• Proficient with Home Care Home Base (HCHB) and OASIS
• Reliable transportation, valid drivers license, and auto insurance
• Excellent clinical assessment, communication, and organization skills
• Ability to manage a flexible, independent field schedule

Preferred:

• BSN
• Experience managing a 2532 point caseload
• Familiarity with Medicare and home health compliance
