Provide skilled nursing care to patients in their homes as a Registered Nurse with experience in home health. This full-time, 13-week contract position requires delivering comprehensive, patient-focused care across a typical caseload of 2532 points weekly (35 patient visits daily) within a field-based setting. You will perform thorough assessments, develop individualized care plans, and collaborate with interdisciplinary teams to support positive patient outcomes.

Key Responsibilities:

• Conduct OASIS assessments and develop tailored care plans.
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize condition changes, and communicate promptly with physicians and care teams.
• Educate patients and families on treatment, medications, and preventive measures.
• Maintain accurate records using Home Care Home Base (HCHB) electronic documentation.
• Coordinate with therapists, social workers, and home health aides to ensure integrated patient care.
• Participate in staff meetings, case conferences, and ongoing education.
• Follow all regulatory guidelines and agency standards for care and documentation.

Qualifications:

• Active RN license in good standing within the state of practice.
• Minimum one year of recent, preferably field-based, home health nursing experience.
• Proficient with Home Care Home Base (HCHB) and OASIS assessments.
• Strong clinical assessment, critical thinking, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently with a flexible schedule.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 points per week caseload.
• Knowledge of Medicare and home health compliance requirements.
