In this field-based role, you will leverage your expertise to deliver compassionate, high-quality nursing care directly to patients in their homes. Your day-to-day experience will be shaped by diverse patient needs and interdisciplinary collaboration, offering you exposure to a variety of clinical practices and leadership styles unique to home health. Youll broaden your resume while making a noticeable difference in your patients lives, all within a supportive travel contract structure designed for both challenge and reward.
Schedule/Hours:
- Contract: Full-time, 13-week assignment
- Monday Friday, 8:00 AM 5:00 PM
- Caseload: 2532 points/week (typically 35 patient visits/day)
Key Responsibilities:
- Conduct OASIS assessments and craft individualized care plans
- Deliver skilled nursing (wound care, IV therapy, medication management, chronic disease support)
- Monitor and document patient progress in Home Care Home Base (HCHB)
- Educate patients and families on clinical care and preventive measures
- Collaborate with interdisciplinary team (therapists, social workers, aides)
- Adhere to regulatory standards, participate in meetings, and maintain compliance
Qualifications:
- Active RN license (PA) in good standing
- Minimum 1 year of recent home health RN experience (field preferred)
- Required expertise in HCHB documentation and OASIS assessments
- Effective communication, critical thinking, and organizational skills
- Reliable transportation, valid drivers license, auto insurance
Preferred:
- BSN
- Experience with similar caseload expectations
- Familiarity with Medicare/home health compliance

$1,897 - $2,007/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
