An experienced Home Health Registered Nurse (RN) is needed to provide skilled nursing care to patients within their homes. This role requires a professional committed to delivering patient-centered care, conducting thorough assessments, and coordinating interdisciplinary care to support positive health outcomes. Key Responsibilities
- Perform comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress and promptly report changes to the healthcare team and physicians.
- Educate patients and families on treatment plans, conditions, medications, and preventive health measures.
- Maintain accurate and prompt documentation using Home Care Home Base (HCHB).
- Collaborate closely with therapists, social workers, and home health aides to ensure seamless care delivery.
- Participate in case conferences, staff meetings, and ongoing education to stay current with best practices.
- Comply with all relevant regulations and agency standards.
Qualifications
- Active RN license in good standing within the state of practice
- Minimum one year of recent home health nursing experience, preferably with field experience
- Proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments
- Strong clinical judgment, communication, and organizational skills
- Reliable transportation with a valid drivers license and auto insurance
- Ability to work independently and manage a flexible schedule in the field
Preferred
- Bachelor of Science in Nursing (BSN)
- Experience managing caseloads of 2532 points weekly
- Familiarity with Medicare and home health compliance standards
Contract Details
- Full-time, 13-week contract
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Caseload of approximately 2532 points weekly (35 patient visits daily)
- Field-based role with daily travel reimbursed
