Deliver exceptional patient care and autonomy as a Home Health Registered Nurse, providing direct support to individuals in the comfort of their homes. This full-time, 13-week contract role emphasizes high-quality, compassionate care, collaboration, and individualized patient engagement within a field-based environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments; develop tailored care plans for each patient.
• Administer specialized nursing interventions: wound care, medication, IV therapies, and chronic disease management.
• Monitor patient conditions, promptly reporting updates or changes to physicians and care teams.
• Educate patients and their families on care routines, medication, lifestyle adjustments, and preventive measures.
• Document assessments and patient interactions using Home Care Home Base (HCHB).
• Coordinate with interdisciplinary teams therapists, social workers, and aides to deliver seamless care.
• Participate in staff meetings, case conferences, and professional development activities.
• Maintain regulatory compliance with all agency and legal standards.

Required Qualifications:

• Current, active RN license in the state of practice.
• At least 1 year of recent field-based Home Health experience.
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS.
• Reliable transportation, current drivers license, and active auto insurance.
• Excellent clinical, organizational, and communication abilities.
• Ability to independently manage a caseload and flexible daily travel within the assigned region.

Preferred Qualifications:

• BSN degree.
• Previous experience managing 2532 points per week.
• Familiarity with Medicare standards and home health regulatory compliance.

$1,970 - $2,084/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
