An exciting opportunity is available for an experienced Home Health Registered Nurse who is passionate about delivering quality patient-centered care in a field-based setting. This contract position offers a chance to work independently while being an integral part of a multidisciplinary care team, ensuring positive outcomes for patients receiving healthcare at home.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized patient care plans.
- Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, promptly identify and communicate changes to the healthcare team and physicians.
- Provide education to patients and families about disease processes, treatment regimens, medications, and proactive care strategies.
- Maintain timely, accurate electronic documentation using Home Care Home Base (HCHB).
- Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to ensure seamless care.
- Participate in case conferences, staff meetings, and ongoing professional development.
- Uphold all regulatory requirements and best practices for home health documentation and patient care.
Qualifications:
- Active RN license in good standing for the state.
- Minimum 1 year of recent field-based Home Health RN experience.
- Proficiency with Home Care Home Base (HCHB) and OASIS.
- Excellent assessment, communication, and time management skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to self-manage a flexible caseload and schedule.
- BSN and prior experience with 2532 points per week preferred.
- Familiarity with Medicare and home health regulatory standards.
