Job Description
Seeking a skilled, compassionate Registered Nurse with a background in home health to provide exceptional patient care in a field-based setting. This full-time, 13-week assignment allows you to work independently while ensuring continuity and quality for patients in their homes. Youll be instrumental in maintaining clinical standards, fostering patient education, and delivering comprehensive skilled nursing care.
Schedule:
- Monday Friday
- 8:00 AM 5:00 PM
Key Responsibilities:
- Conduct comprehensive OASIS assessments and establish individualized care plans.
- Deliver skilled nursing services, such as wound care, IV therapy, medication administration, and chronic disease management.
- Evaluate patient progress, promptly communicate changes with care teams, and coordinate with physicians.
- Educate patients and their families on treatment plans, medications, and preventive care.
- Document care activities accurately and efficiently using Home Care Home Base (HCHB).
- Collaborate with the broader interdisciplinary team, including therapists, social workers, and aides, to ensure coordinated care.
- Participate in case conferences, staff meetings, and ongoing educational opportunities.
- Maintain compliance with all regulatory and agency standards.
Requirements:
- Active RN license in good standing
- At least one year of recent home health nursing experience (fieldwork preferred)
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Reliable transportation, valid drivers license, and auto insurance
- Strong assessment, organizational, and communication skills
- Ability to work independently in a flexible, field-based environment
Preferred:
- BSN
- Experience handling a caseload of 2532 points weekly
- Familiarity with Medicare and home health compliance standards
