Seeking a motivated and compassionate Registered Nurse with home health experience to deliver high-quality patient care in a field-based setting. This full-time, 13-week contract offers a Monday through Friday, 8:00 AM 5:00 PM schedule, providing work-life consistency for skilled clinicians seeking meaningful impact. Expect a caseload of 2532 points per week, typically 35 patient visits daily. Youll conduct daily travel within an assigned territory, with mileage reimbursement to support patient-centered service.

In this role, your responsibilities will include:

• Conducting comprehensive patient assessments using OASIS and developing individualized care plans.
• Delivering skilled nursing interventions, such as wound care, medication administration, IV therapy, and chronic disease management.
• Evaluating patient status, promptly reporting changes to the care team and physicians.
• Providing patient and family education on medication, disease process, and preventive care strategies.
• Documenting all care provided within Home Care Home Base (HCHB).
• Collaborating with interdisciplinary professionals (PT, OT, MSW, HHAs) to deliver coordinated, exceptional care.
• Participating in case conferences, staff meetings, and ongoing professional development.
• Adhering to all regulatory and agency standards for documentation and care delivery.

Position requirements:

• Active RN license in good standing for the state of practice.
• Minimum 1 year of recent field-based home health RN experience.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical assessment, decision-making, and organizational abilities.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to manage a field-based schedule independently.

Preferred: BSN, prior caseload management of 2532 points per week, and familiarity with Medicare/home health compliance.

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
