We are looking for an experienced and compassionate Registered Nurse (RN) who is passionate about delivering high-quality care and supporting patients as they recover in a familiar environment. This is a 13-week, field-based contract role with an immediate need.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, such as medication administration, wound care, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes in condition, and promptly notify care teams and physicians.
• Provide education to patients and families about their health conditions, treatment plans, and medications.
• Document all patient care accurately and efficiently in Home Care Home Base (HCHB).
• Collaborate with a multidisciplinary team including therapists, social workers, and aides.
• Participate in meetings, training, and continuing education as needed.
• Adhere to regulatory standards and ensure compliance with agency protocols.

Requirements:

• Active RN license (in good standing in the state of practice)
• Minimum 1 year of recent home health experience (field preferred)
• Proficiency in Home Care Home Base (HCHB) and OASIS required
• Strong clinical assessment and critical thinking skills
• Reliable transportation, valid drivers license, and auto insurance
• Excellent communication, time management, and organizational skills
• Ability to work independently with a flexible field-based schedule

Preferred:

• BSN degree
• Experience managing a weekly caseload of 25-32 points
• Familiarity with Medicare and home health compliance
